Challenge
Traditional healthcare apps focus only on tracking or storing health records, but lack personalization and structured care guidance. Patients often feel lost between multiple apps and scattered prescriptions, while doctors struggle to maintain continuous engagement with their patients.
The challenge was to create a platform that bridges this gap enabling doctors to build structured care frameworks (like Pregnancy Care, Childcare, or Mental Wellness), and allowing patients to seamlessly access them in one place.
Results
After rollout and testing with pilot users, we observed:
40% increase in patient adherence to doctor’s prescribed frameworks.
30% improvement in communication between doctors and patients.
4.7★ average user satisfaction from early testers.
Reduced drop-off rate by 25% due to simplified onboarding and navigation.

Process
Research & Analysis: Research & Analysis Conducted interviews with doctors and patients to understand pain points in current health apps. Identified that doctors wanted control over frameworks, while patients wanted simpler guidance.
Information Architecture: Created two distinct platforms: one for doctors (HD Create) and one for patients (HD Connect). Focused on prioritizing ease of use and quick access to personalized care plans.
Wireframing & Prototyping: Built low-fidelity wireframes to map doctor workflows and patient journeys. Iteratively tested with doctors to ensure framework creation was seamless. Designed interactive prototypes for patients with step-by-step task flows.
Usability Testing: Conducted multiple usability rounds with both patients and doctors. Simplified onboarding flow for patients and added “framework preview” for doctors.
Visual Design & Style Guide: Adopted a calm, trust-oriented color palette (Lime Yellow & Purple). Created a consistent design system with typography, icons, and layout guides. Ensured accessibility compliance for patients of varying age groups.
“ With Health Diary’s new design and patient-first approach, we’ve built a platform that truly bridges the gap between doctors and patients.
